1. BUSY ED - excellent Docs there. You will get 5 admissions at once ( and VERY sick patients) 2. Floor Nurses inexperienced ( or experienced, but learned poor management skills), defy orders, page the MD for small and inconsequential things. They cannot tell a "sick pt." from a "not sick" pt. You will get 5 pages w/in 8 mins, insisting that you come to the bedside RIGHT NOW. Nurse won't even be able to give you the HR,BP, UOP. 3. CEO forbids ED from transferring a pt. elsewhere. Thus, pt. will sit in hall or a room across from the ED. w/ no Nurse monitoring them ( this incl DKA, AKI w/ K+ 6, etc..). As admitting Hospitalist, you are responsible for this pt.. 4. ICU is EXCELLENT. Very bright, experienced Intensivists. They adhere to Leapfrog - Intensivist presenty there 24 hrs/day, good continuity of care. 5. Unhappy, depressed, cynical Hospitalists. One of them in particular vigorously fights w/ ED physicians about admissions. His MO is to threaten any colleague w/ "I'll report you to Peer Review". He is famous among other local hospitals - his Chief and CEO do not acknowledge the problem, or intervene. He often prematurely discharges pts., so you get a lot of his bouncebacks. Hospitalists are so overwhelmed that there is no collegiality, - a mercenary "every man for himself" atmosphere. 6. "Doctor Lounge" is archaic. A small room, no sink there to wash your hands. All hospital employees enter it 24 hrs/day, have lunch, take home food put into 'fridg by kitchen staff. The only food avail. for Nocturnists is crackers, Lorna Doone cookies, and chips. Also soda. 7. They have gone through several CMOs and CEOs.
